Please see announcement below concerning a new form required for all MINORS attending Encampment who will be bringing medication:
It has come to our attention that the State of Maryland has made significant changes to the medication administration policy for minors attending camps in our State. Please have your Primary Care Providers complete a Medication Administration Authorization Form (found here) for each prescription and non-prescription medication that your child will need during camp. In addition, they are requiring that the Primary Care Provider and parent/guardian authorize supervised, self-administration of medications, including emergency medication such as an Epi Pen or Inhaler. Because of the short notice you may bring these additional completed forms with you to Encampment and turn them in during sign-in. If you would like to send the forms in advance, you may scan them and forward them to 1st Lt Dean by email at firstname.lastname@example.org. If you have any questions or concerns you may contact 1st Lt Dean by email or Lt Col Hiles at email@example.com. We apologize the last minute change and appreciate your time. We'll see you at Encampment!
